Certificate/Statement for childcare, kindergarten or school enrolment
An immunisation certificate/statement will be requested by Childcare centres, kindergartens and primary schools on enrolment.
This must be an Immunisation History Statement from the Australian Immunisation Register (AIR). The AIR routinely sends out an updated immunisation certificate to parents of all four-year-old children after they have received their 4-year-old vaccination.
How to get an Immunisation History Statement from the AIR
- Print a copy of your child’s Immunisation History Statement via your individual myGov account
- Request a copy by contacting the Australian Immunisation Register (AIR) on 1800 653 809
- Request a copy by visiting a Medicare or Centrelink office
- If you do not have a Medicare card you must phone the AIR on 1800 653 809 to request an Immunisation History Statement be sent to you
Children vaccinated overseas

Has your child had vaccines overseas and you need an immunisation certificate for kindergarten, childcare or Centrelink?
Children need to provide an immunisation history statement that shows their child’s vaccinations are up to date for their age to enrol in kindergarten or childcare or to receive some Centrelink payments. You will need to complete the Immunisation History Assessment Form below.

You must provide an Immunisation History Statement to a primary school when enrolling your child. You will need to complete the Immunisation History Assessment Form below.

- Complete the Immunisation History Assessment Form below for each child.
- A fee may be charged to provide this service – After the form is submitted you will be contacted for payment prior to the service being commenced. The fee is advised on the form.
- You will be asked to provide vaccination records, these must be in English.

- A certificate/statement from the Australian Immunisation Register (AIR) will be provided after your child’s vaccination history from another country, or provider, has been assessed, and the vaccines recorded on the AIR.
- A vaccination catch-up plan for any vaccinations needed to bring your child up to date for age can then be provided by Council or you can choose to go to your doctor.
